Standard Patient Condition Reports
University Community Health uses the following standard definitions to describe patient conditions. The use of these standard terms in responding to media inquiries of a specific patient’s condition will comply with HIPAA requirements, so long as additional information is not included.

Undetermined -
Patient is involved in a procedure and/or awaiting assessment.

Good -
Vital signs such as pulse, temperature and blood pressure are stable and within normal limits. Patient is conscious and comfortable. The outlook for recovery is good or excellent.

Fair -
Vital signs are stable and within normal limits. Patient is usually conscious, but may be uncomfortable or have minor complications. Favorable outlook.

Serious -
Vital signs may be unstable or not within normal limits. Patient is acutely ill with questionable outlook.

Critical -
Vital signs are unstable or not within normal limits. There may be major complications. Patient may be unconscious and death may be imminent.
NOTE: Although often used, the terms "Stable" and "Guarded" do not have universally accepted definitions and, therefore, are not used by University Community Health.
University Community Health may also indicate that a patient has been treated and released, or treated and transferred.
